New Patients are always welcome at Reynella Medical Centre.
If you are a new patient, you can save time by printing and completing our New Patient Form below and bring along to your first appointment.
If you would like your Medical Records to be transferred to us please complete our Transfer of Medical Records Form below and bring along to your first appointment this will then allow us to obtain records from your previous GP clinic.
Where possible please book a longer appointment for your first visit so your Doctor can discuss your medical history and obtain up to date information. Alternatively book a time with one of our Nurses prior to your next Doctor appointment.
Please be aware that some Doctors who are transitioning to retirement may only see their existing patients.
